TapRooT® RCA is a systematic process, software, and training for finding the real root causes of audits, precursor incidents, or major accidents.

The TapRooT® System is used by leading companies around the world to investigate and fix the root causes of major accidents, precursor incidents, quality issues, human errors, maintenance problems, medical mistakes, productivity issues, manufacturing failures, environmental releases, audit findings … in other words, all types of mission-critical problems.

TapRooT® RCA has been designed for investigating seemingly simple problems that could have been worse (precursor incidents) by using a simple 5-Step Process. TapRooT® RCA can also be used to investigate major accidents by using the complete 7-Step Process.

  • The 2-Day TapRooT® Root Cause Analysis Course teaches the essential techniques for investigating precursor incidents and fixing them to prevent major accidents.
  • The 3-Day Virtual TapRooT® Root Cause Analysis Course teaches the same techniques as the 2-Day TapRooT® Course but virtually.
  • The Equifactor® Training also teaches how to use the TapRooT® Techniques and Equifactor® Troubleshooting Tables to stop equipment problems.
  • The 5-Day TapRooT® Root Cause Analysis Team Leader Course teaches the complete 7-Step TapRooT® Process and Techniques to investigate major accidents, serious environmental releases, fatalities, major plant upsets, serious injuries as well as precursor incidents.

Both simple investigations and major investigations are supported by the TapRooT® Software. You can switch from the 5-Step Process and essential techniques to the 7-Step Process and all the techniques by toggling a switch.

History

30 years of research, development and continuous improvement.

Over 30 years ago, we started with research into human performance and the best incident investigation and root cause analysis systems. We put our knowledge to use to build a systematic investigation process with a coherent investigation philosophy. Then, we used and refined the system in the field. We’ve performed formal user testing and informal focus group appraisals. In 1991, we wrote the first TapRooT® Manual that put all of this experience together into the best incident investigation system – TapRooT®!
