Some accidents are so historic that every accident investigator should know about them. The Challenger is one of those. It happened 24 years ago today. The sad part is that NASA knew they had a problem with the seals due to 13 previous burn throughs, but they would not consider “fixing” the problem. Their mentality (apparently) was to either keep flying or take a Major setback in time and money to redesign the SRB Seals. They could have applied a “fix” to keep flying Safely while a redesign was done. The fix could have been a shield between the SRB joints and the Fuel tank. The shield would only have to last for 3 minutes, and it would have taken a few thousand pounds off of the payload but it would have allowed the shuttle to keep flying Safely.
