The weekly format seems to be popular (not too many e-mails) and the joke is usually the most clicked on item. However, people also seem interested in accident news, root cause analysis tips, and the weekly “How Close to Death” picture or video.

Hello Mark,
As a relative novice to RCA, having moved into QI/Patient Safety 3 years 30+ years in nursing, NP practice and nursing administration, I am very interested in RCA. I initiated RCA in the hospital I work in Saudi Arabia. We have done 3 and started shorter RCA’s from a british format. We are experimenting with different methods but a comment I read a short time back on this site regarding the questionable benefit of drilling down with ‘why’ and how it breaks the train of thought resonated with me. With our RCAs I am left with the nagging feelings of ‘did we get it right’. Our efforts are leading to changes and opening eyes, (that often seem would rather stay shut), however, I want to develop my RCA skills. I hope to take one of your courses in the future but an currently preparing for the CPHQ exam. I would like to see more health care related RCA material. Another area of concentration is PI and I am currently studying and introducing the model for improvement. Risk management and PI are my two interests/needs and of course, working with data. I guess that covers most things.

Great article and pictures
I have used the word “death” in safety training and it seems to click on the attendees switch on OFF, would like to suggest another word or term to enlighten.

I would like to see more examples of actual root cause analysis conducted with TapRoot. Surely this could be done without giving away all the secrets ; ). I would also like to see a side by side comparison of analysis using TapRoot and other popular methods. This would not only benefit your company but those of us who have yet to attend your training would be able to use this kind of info as justification for attending.
Finally, my company does not permit access to YouTube or Facebook so I am unable to see many of your videos. Why not host these yourself? I have to think that Honda isn’t the only large company blocking access to social media sites.
