Brazil is known for spectacular beaches, the flamboyantly colorful Carnival, and its symbolic statue of Christ the Redeemer at the peak of Corcovado Mountain.

    • Brazil is also home to Sao Paulo, the financial hub of Brazil and where world-renowned Pele played at the legendary stadium Pacaembu.

    • Did you know each Brazilian city has at least one soccer stadium?
  • It’s been called “one of the most beautiful bridges in the world” and “the bridge with the complicated name”—Octavio Frias de Oliveira bridge, or Ponte Estaida, over the Pinheiros River in Sao Paulo is known for its unique architecture. This national landmark is the only bridge in the world with an X-shaped supporting structure, with the cruciform prop supporting two crossing roads of different heights. The bridge’s nighttime lights are spectacular.

All the furniture, mosaics, and sculpture were transported by ship from Italy.

Patio do Colegio, or “Courtyard of the College,” is Sao Paulo’s oldest building and a historical Jesuit church and school. The Pátio do Colégio is home to the Museu Anchieta, which features a fascinating collection of artifacts and records from the 16th century.

Monday Accidents & Lessons Learned: A Taxiway by Any Other Name

July 23rd, 2018 by

An EFB, or electronic flight bag, is portable electronic hardware, increasingly utilized for flight deck or cabin use to facilitate flight crews perform flight management tasks easier and more efficiently. At a basic level, EFBs can perform flight-planning calculations and offer a variety of digital documentation, such as navigational charts, operations manuals, and aircraft checklists.

NASA’s Aviation Safety and Reporting System (ASRS) has received reports that describe various kinds of EFB anomalies. This report illustrates how complications between EFBs and human operators develop into precursor events:

“A B737 Captain encountered frustration while using his moving map. Although the specific incident is not cited, the Captain clearly identifies an EFB operational problem and offers a practical solution for the threat.

‘In [our] new version of [our EFB chart manager App], … a setting under Airport Moving Map (AMM) … says, “Set as default on landing,” [and I cannot] … turn it off. If [I] turn it off, it turns itself back on. This is bad.… It should be the pilot’s choice whether or not to display it at certain times—particularly after landing. Here’s the problem with the AMM: When you zoom out, the taxiway names disappear.

‘Consider this scenario: As you turn off the runway at a large airport, you look down at the map (which is the AMM, not the standard taxi chart, because the AMM comes on automatically, and [I] cannot turn that feature off). You get some complicated taxi instructions and then zoom out the AMM [to] get a general, big-picture idea of where you’re supposed to go. But when [I] zoom out the AMM, taxiway names disappear.… [I] have to switch back to the standard taxi chart and zoom and position that chart to get the needed information. That’s a lot of heads-down [tablet] manipulation immediately after exiting the runway, and it’s not safe.

‘[Pilots should have] control over whether or not to automatically display the AMM after landing. The AMM may work fine at a small airport, but at a large airport when given taxi instructions that are multiple miles long, the AMM is useless for big-picture situational awareness.'”

The award-winning publication and monthly safety newsletter, CALLBACK, from NASA’s Aviation Safety Reporting System, shares reports, such as the one above, that reveal current issues, incidents, and episodes.

Circumstances can crop up anywhere at any time if proper sequence and procedures are not planned and followed. We encourage you to learn and use the TapRooT® System to apprehend situations and find and fix problems. Attend one of our courses. Among our offerings are a basic 2-Day Course and an advanced 5-Day Course. You may also contact us about having a course at your site.

Be sure to discover even more points of interest to explore on our Dubai Pinterest board.

Experience Dubai

Dubai is an exhilarating blend of history and culture, panoramic views, world-class beaches and shopping, and unforgettable experiences. A constitutional monarchy, established  in 1833, Dubai today is a gleaming metropolis that boasts the world’s tallest building—provides a stark contrast to the setting of Old Dubai, or Old Town, with its coral-hued, low-rise structures. You can learn about the traditions and culture of Dubai here

The 2,722-foot skyscraper Burj Khalifa is the world’s tallest structure, its design influenced by Islamic architecture. Burj Khalifa is surrounded by a 27-acre park with pools and fountains in the park’s center. Watch the impressive Dubai Fountain, with 6,600 lights, 50 projectors, and 35 melodies among its choreography, water from the fountain dancing 500 feet into the air. Buy tickets in advance for the best price for At the Top observation decks, the fountain boardwalk, or the fountain lake ride. Plan to have dinner at an outside table near the Dubai Mall at one of the Souk Al Bahar restaurants for a good view of the fountain show, which occurs every half hour from 6:00 pm to 11:00 pm.

With the Palm Islands, Dubai is underway in developing remarkable man-made archipelagosPalm Jumeirah, Deira Islands, Palm Jebel Ali, The World, and Bluewaters Island.

Savor the fresh air on Palm Jumeirah: Rent a bike and go exploring.

Cool off and delight in a water adventure.

Experience Old Town and its enchanting souksA “souk” is a market. Dubai Gold Souk is the duty-free gold market in historic Deira, or Old Dubai, offering some of the world’s lowest prices in gold jewelry. Nearby Heritage Village and Diving Village will give you glimpses into an earlier Dubai culture through crafts and the history of the pearl industry.

You’ve seen silhouette images of this icon: the alluring, sail-shaped Burj Al Arab Jumeirah. Now, get a close-up view of this graceful monument that offers resplendent hospitality to its guests.

Visit Jumeirah Mosque. The mosque is open to non-Muslims and a tour offers the opportunity for insight into the Islamic religion.

Note: The Dubai Metro is a great way to get around the city, and the car windows are huge to facilitate spectacular views.

Taste Dubai

Tuck into Turkish coffee, wonderful teas, dates with ice cream cake with chocolate fondue, a lovely breakfast—all with a splendid view of the Palace Hotel—at La Postreria.

Sample the seafood at Bice Mare on the terrace while overlooking Burj Khalifa and the Dubai Fountain.

Order scrumptious Italian fare at Serafina Dubai.

Al Dawaar: This revolving rooftop eatery, Al Khaleej Road, Deira Corniche, is on the 25th floor of Hyatt Regency Dubai.

Zaroob: Dine on Arabic street fare in the outdoor garden near Dubai International Financial Centre.

See the souks, the Dubai Courts, and all the sights from a delicious dinner cruise on the Bateaux Dubai.

Monday Accidents & Lessons Learned: When One Good Turn Definitely Doesn’t Deserve Another

July 16th, 2018 by

The electronic flight bag (EFB) is rapidly replacing pilots’ conventional papers in the cockpit. While the EFB has demonstrated improved capability to display aviation information—airport charts, weather, NOTAMs, performance data, flight releases, and weight and balance—NASA’s Aviation Safety and Reporting System (ASRS) has received reports that describe various kinds of EFB anomalies, such as this one:

“This B757 Captain received holding instructions during heavy traffic. While manipulating his EFB for clarification, he inadvertently contributed to an incorrect holding entry.

‘[We were] asked to hold at SHAFF intersection due to unexpected traffic saturation. While setting up the FMC and consulting the arrival chart, I expanded the view on my [tablet] to find any depicted hold along the airway at SHAFF intersection. In doing so, I inadvertently moved the actual hold depiction…out of view and [off] the screen.

‘The First Officer and I only recall holding instructions that said to hold northeast of SHAFF, 10-mile legs. I asked the First Officer if he saw any depicted hold, and he said, “No.” We don’t recall instructions to hold as depicted, so not seeing a depicted hold along the airway at SHAFF, we entered a right-hand turn. I had intended to clarify the holding side with ATC, however there was extreme radio congestion and we were very close to SHAFF, so the hold was entered in a right-hand turn.

‘After completing our first 180-degree turn, the controller informed us that the hold at SHAFF was left turns. We said that we would correct our holding side on the next turn. Before we got back to SHAFF for the next turn, we were cleared to [the airport].'”

Volpe National Transportation Systems Center, U.S. Department of Transportation, weighs in on EFBs: “While the promise of EFBs is great, government regulators, potential customers, and industry developers all agree that EFBs raise many human factors considerations that must be handled appropriately in order to realize this promise without adverse effects.”

CALLBACK is the award-winning publication and monthly safety newsletter from NASA’s Aviation Safety Reporting System (ASRS). CALLBACK shares reports, such as the one above, that reveal current issues, incidents, and episodes.

Circumstances can crop up anywhere at any time if proper sequence and procedures are not planned and followed. We encourage you to learn and use the TapRooT® System to find and fix problems. Attend one of our courses. We offer a basic 2-Day Course and an advanced 5-Day Course. You may also contact us about having a course at your site.

Recently, on TapRooT®’s Facebook Live, we learned that only through effective listening will you learn to pick up on the “right” questions to ask in your investigations. TapRooT® Instructor Barb Carr gave us a beginning point:”The first question is the only one you need to know going in: ‘Tell me, from start to finish, what you observed the day of the incident.’” Barb also advises that the next step is to “sit back, listen, and identify which follow-up questions need to be asked.”

Since our listening skills develop with practice, everyone can use help becoming better investigators. Use the video and Vimeo below, featuring TapRooT® professionals Benna Dortch and Barb Carr, to review your skills:

Visit Bogota and discover an attractive, vibrant city abuzz with great coffee shops, culinary hotspots, a wealth of heritage and cultural high points, and colorful neighborhoods. Colombia has the world’s largest number of orchids, its national flower. In Bogota’s mountains, you will find a diversity of these orchids, as well as a wealth of species of flowers, birds, and butterflies. Below are some suggestions and highlights of Bogota for you to explore:

El Carmen Church (Iglesia Nuestra Señora del Carmen): You’ll easily spot the iconic 187-foot, red-and-white brick steeple in the Candelaria district. Decorated with Arabic and Byzantine art, this Gothic-style church was built between 1926 and 1938 with architecture that reflects the Spanish colonial tradition. See other churches in Candelaria here.

Plaza de Bolivar: A visit to Bogota often begins here, in the heart of the original town. In this square is the city’s first public monument, the 1846 bronze statue of Simon Bolivar, known as El Libertador.

La Catedral PrimidaGothic-style with a French influence, the Holy Cathedral Church has five naves, 88 columns, and 72 vaults.

The Gold Museum (Museo del Oro), in the cobblestoned La Candelaria, houses the largest collection of pre-Colombian gold work in the world. Among the 55,000 pieces of ancient works by indigenous cultures of Colombia, you’ll see gold, pottery, rock, precious stones, and textiles, lending scientists and archaeologists insight into the cultures of South America civilizations prior to the arrival of Europeans.

Visit La Candelaria in daylight hours. You’ll find this the place to experience Bogota’s colorful, cultural heritage, as the neighborhood is full of art deco, Spanish colonial, and Baroque architecture. La Carrera Septima is a good beginning for wandering your way to La Candelaria.

Take in panoramic views of the city and faraway hills atop Monserrate Mountain, a 10,341-foot mountain with a 17th-century church, gardens, and a Colombian spiritual shrine. Ascend to the mountain by cable car, railway, or on foot. The cable car views are preferred by most visitors. On the mountain, you will also find a couple of restaurants and a market.
