Billy, what did you think about the Summit Session, OSHA: Be Prepared for Sweeping Workplace Safety Changes?

“The former leader of OSHA under the Bush administration put an emphasis on safety of how important it was to you and me and to our company’s, and how we need to get passionate about being safe. Everything we do, every single day, not only in our workplace but at home, is so important to do it safely so that we can continue on.”

What were other sessions you enjoyed, as well?

“We just got done with the session on Body Language and how to interpret someone’s body language. It’s not just the interviewing and getting the answers but also finding out what their body is telling you when doing an interview. It is an important part in an interview investigation to find a root cause of something.”

What do you think about the Summit?

“I think the Summit is absolutely awesome. I had a great time. Learned quite a bit of stuff. Networking with hundreds of different people with what they do and what works for them and what’s not working for them and finding out that our problems are not just our problems. Most everybody is going through some of the same things.”

So, what would you say to your co-workers about the Summit?

“Yes, we need to come to the Summit more often since this is my first one.”

Have you learned any best practices that you’ll be taking back? 

“Yes, we had a couple of really good ideas, actually. The best practices that we created over the last couple of months, we are going to add going into corrective actions and digging deeper into tracking corrective actions. Not just talking about TapRooT® itself, but how effective are our corrective actions that we’re coming up with?”

What is your favorite part about the Summit? 

“My favorite part this year was how all the presentations worked together to build a theme and, for me, that was team work and getting your corrective actions done and using the TapRooT® method to figure out what you were doing.”

2014 Global TapRooT® Summit Attendees Share Favorite Aspects from the Summit

Rich Horan from PPL Electric Utilities shared some of his favorite aspects of the 2014 Global TapRooT® Summit in Horseshoe Bay, TX. Listen to what content he enjoyed most and what he will be taking back to his workplace to implement some of the Best Practices shared at the Summit.

How did you like the Summit? 

“I loved it! Very well organized. It was my first Summit and from the beginning right up to the end today it was very enjoyable and very nice to meet a lot of the people looking for the same experience and networking.”

What was your favorite session? 

“I would have to say the Best Practices and Learning. It was very interactive but there was a lot we walked away with, that I walked away with that get to take back to my work place and apply some of those lessons learned. So, I would have to say that was the better session of all of them.”

Can you think of any of those Best Practices that you’d like to share with us?

“The corrective actions and the effectiveness reviews. So the quality of the investigation is important but most important is the corrective actions and the effectiveness review as far as a follow up to see if there is some quality in those investigations.”

Did you attend the reception? How did you enjoy that? 

“Well, it was absolutely wonderful. It was a good way to break away and meet some people. There was beautiful weather, good music, and probably one of the better receptions I’ve been to in some of the conferences over the years, so, well done!”

How was the networking and the people that you met here? 

“I think that networking is very important and from that, learning what others are doing and sharing what I have done in the past two years as a TapRooT® instructor. So, very valuable to meet other people and continue that networking after the conference.”

How did you all like the Summit?

“The summit was a great opportunity for the team to build on their skills, remind us of some of the skills we forgot, provide an opportunity for all of us to get on the same page and move the TapRooT® program events forward.”

What were some of your favorite sessions?

“One of the sessions that I attended was with a really good speaker. He brought a lot of information, not just about how we do our job, but how we do our jobs more effectively and how we take that home to our families and how to better each one of us as a whole. It was very good.”

What was one of your favorite best practices that youre going to bring back to work with you?

“We, as a team, are going to share all of our TapRooT® Investigations with each other. We’re all from different regions all over Canada, Pennyslvania and North Dakota. By getting incidents, sharing it across the board, we’re going to be able to bounce ideas off of each other and find out, well “I’ve had this happen before and this is what we did” and our corrective actions are going to get a lot better because of it.”

Tell me a little bit about the reception.

“The reception was great. It was a great opportunity to get out, network, meet other companies, hang out with each other, watch some good music and really just relax at the end of the day. It was nice.”

Tell me a little bit more about this week’s team building exercise.

“This was a great opportunity since our team is spread across many different regions. It’s the first opportunity we’ve had to get the full team together. We had some exercises with some questionairres assigned and some fun assignments were assigned to group members throughout the week. It really brought us closer as a team. We had the opportunity to share some laughs and attend a lot of good sessions.”

5 Reasons You Should Attend New Delhi TapRooT® Training

We held our first TapRooT® course in India in February, and it was a huge success. We’ve decided to hold another 5-Day course, this time in New Delhi.

  1. It’s the first time ever! September 5-9, 2011 is our first 5-Day TapRooT® Advanced Investigation Team Leader Training in New Delhi, India. This is our last scheduled India course in 2011, so register fast!
  2. The last one sold out. We held a 5-Day TapRooT® Training in Mumbai in February, and it was incredibly popular. The attendees really enjoyed it, saying that the course was “a very good tool for investigation and finding root cause(s).”

Summit – Day 2

I’m getting behind on my Summit reporting because of all the amazing talks and amazing discussions I’m having. I just can’t keep up.

Here are some of the highlights…

Success Stories: I attended a session with two success stories yesterday. Ron Pryor (Alcoa) and Theresa Guay (Irving Oil) presented their stories of how they were using TapRooT® to get significant improvements in environmental, quality, and safety performance. We’ll post the Success Stories at when we get back from the Summit, so you can look for the details there. But what you can’t get from the written word is the Q&A that occurs at the session. For that, you must attend the Summit.

Personal Development: Next, I went to a great presentation by Jennifer Mounce that was very interactive. She taught us about coaching ourselves to get better and bring our personal performance to the next level. Once again, you had to be there to participate and learn. It really gave me some good ideas to act on to improve my own performance.

Lessons from Success: Then we had an inspiring General Session talk by E.D. Hill. Wow! E.D. is smart, insightful, and a great speaker about lessons from the success of others and her own career. After her talk she spent and hour and a half with us signing her book and talking to people one-on-one. She even came to our reception where the band Entice entertained Summit participants.

Lessons from an Accident: This morning I heard an amazing talk about an injury, the aftermath, and the investigation that changed the standard of care for electrical injuries in the province of Ontario. Jim Thompson, Brian Tink, and Dr. Joel Fish shared their story which was very impressive. We had to cut off the Q&A and let people continue the discussion at the break.


Equipment Failure Lessons Learned: I’m now in a talk about “Persistent Equipment Failures” by Ken Bloch. He’s talking about equipment failures that cause process safety incidents. One neat detail so far … He quoted a statistic that process plants could expect one fire per 1000 pump repairs.
