What if you had hundreds of texts on your personal cell phone? Would it be OK to back them up to your computer and then delete them? That's what I heard the negineer did (though I'm not certain it is true.)

I'm in government (but I'm not a bureaucrat!!) - serving a highly vulnerable population. The requirements say within 24 hours - sounds simple, right? NOT :-) When I explored with the team how this was actually implemented in the field, I learned about several scenarios that can actually delay this by days!! In the meantime, precious info is lost, staff collaborate on "stories" before being interviewed, the site is not secured, etc. One of my mantras: it's easy to aspire to mediocrity!!

Treat every investigation as a crime scene and every scrap of evidence as if the entire case hinges on that one scrap. Leave it to someone else to determine what is not necessary and what can be destroyed - like your company legal team. Safety professionals and investigators should take the high ground and the safe road when it comes to preserving evidence. Its not my job to say this or that is no longer of value.

Watching this accident, the first thing that came into my mind was why the fencing was so close to the course border. Whether it was human error or not, we in the safety industry do Hazard and Risk Assessments to protect the individuals to include human error. Not knowing what the fence posts were made of keeps me from commenting on the construction portion of the course. I do however believe the fence did not factor in human error. Like all sports, the closer the fans are to the entertainment, the better the sport prevails because as a spectator, you want to be close. We have Nascar Drivers who can flip and roll a vehicle at 200mph and walk away, yet we cant control skiiers from being killed. Contruction should be able to take a hard hit from the skiiers, giving way after so much force as to reduce the amount of blunt trauma. Personally, I believe this fatality was easily preventable.

With historical knowledge of potenial hazards the fencing should have been designed to reduce the effects of impact.
