Ralph Blessing possesses over 25 years of safety and health experience encompassing general industry, construction, training and public speaking.  Ralph began his safety career while in the United States Navy stationed on Diego Garcia, in the Indian Ocean.  As the base safety officer he was responsible for the 5000 military and civilian personnel that were permanently assigned to the island, in addition to those transiting the area. 

Upon retiring from the U.S. Navy, Ralph accepted employment in Saudi Arabia where he was a technical trainer at the Royal Saudi Air Force School of Command, Control and Communications. Ralph was also the first corporate safety director for McKenneys, Inc. mechanical contractors and built the safety program from the ground up.  Within his first 6 years McKenneys, Inc. achieved 1 million hours of work without sustaining a lost time injury on three separate occasions. Ralph continues to advise McKenneys on safety issues.

Ralph possesses a M.S. in Occupational Safety and Health and is an adjunct professor at both Southern Polytechnic State University in the Construction Management Program and at the College of Safety and Environmental Studies at Columbia Southern University.  In addition, he has conducted safety classes that have trained in excess of 4000 personnel.  He was the first safety professional appointed to the Sheet Metal Occupational Health Institute Trust (SMOHIT) as a trustee and sat on the SMACNA safety committee.

He has represented the University of Tennessee in facilitating two seminars on Action Research in Thailand.

Ralph lives in Kingston, Tennessee with his wife Janice. Ralph enjoys traveling and exploring out of the way places. He is very interested in amateur archaeology and has visited the Angkor Wat complex in Cambodia, Authyia in Thailand, the Khyber Pass in Pakistan/Afghanistan, and most of the major pyramids and sites in Mexico and Central America. He enjoys playing with his three young grandsons and introducing them to bicycles, motorcycles, jeeps, caves, cliffs and kayaks, just a few of his many and varied interests. He frequently rides long distance bicycle tours both solo and with groups. His longest ride was a 3,000 mile Pacific to the Atlantic coast ride in 27 days.

Janis Chartier is manager of PPL Electric Utilities Experience Assessment Group.  PPL Electric Utilities (EU) is a PPL subsidiary that delivers electricity to about 1.4 million customers in central and eastern Pennsylvania. 

Chartier manages several of the performance improvement processes at PPL Electric Utilities including the event investigation and corrective action programs, benchmarking, behavior-based observations, and operating experience.  Chartier is also responsible for implementation of the human performance program at PPL EU.

Chartier has more than 20 combined years of military and utility industry experience.  Chartier became the manager of the PPL EU Experience Assessment Group in September 2007 after leaving Exelon Nuclear.

Previously, she was the Limerick Generating Station Self-Assessment, Benchmarking & Operating Experience Coordinator for Exelon Nuclear.  Earlier in her career, she held several positions with increasing responsibility in naval public relations, industrial safety, power plant operations and performance improvement.

Dr. Chiodo is a professor in the Department of Management at Texas State University.  Dr. Jerry Supple, former President of Texas State, said she has won every teaching award the university offers, and she's been recognized for her teaching effectiveness at the state and national levels as well.  In 1996, she was featured as "Hero of the Day" by CBS national TV program -- This Morning.  In 1997, Beverly was named "The Best Business Professor in the Nation" by the National Business Education Association.  She has published widely in her field and is a frequent presenter at seminars and conferences.

Eric Cropp
Eric Cropp has sixteen years of experience working in the field of pharmacy, the majority of that time in the oncology field. For several years he managed a compounding pharmacy that specialized in the treatment of cancer patients, where he managed patients’ care from their cancer treatments to supporting their pain management. Shortly before the incident that is the subject of this program, he had begun finishing his PharmD degree, with which he intended to specialize in pediatric oncology.
 
Mr. Cropp was a pharmacist at an Ohio Hospital on February 26, 2006, when a pharmacy technician under his supervision made an error in mixing a saline/chemotherapy solution intended for two-year-old Emily Jerry. Emily, who was undergoing treatment for a spinal malignancy, died three days after the solution was intravenously administered. Mr. Cropp was ultimately convicted of involuntary manslaughter.
 
Since finishing his sentence of six months in jail and six months of home arrest, Mr. Cropp has been working with several organizations (ISMP, PharmCon) telling his and Emily’s stories. He has also lectured to several large companies about the importance of patient safety as well as of safe conditions in the work place. He shows his audience all the areas at Rainbow Children’s that could have been improved to ensure a safe outcome. He has recently begun to work with Chris Jerry, Emily’s father, to further teach families and medical professionals the importance of creating the safest environment for both the patient and the caregiver.

From his youth, Marco Flores has dedicated himself to exploring the limits. During his school days in 1965, he was a member of the International Society of Interplanetary Studies, and the University Society for Experimental Rocketry. While at M.I.T. coursing a Master’s Degree in Aeronautics and Astronautics, he was part of the Innovation Center and the Commission for Hispanic Rights, dedicated to defend the human rights of the Spanish-speaking prisoners in Massachusetts. While at the Innovation Center he produced the famous “Phantom Potential Racer” and the “Automatic Banana Peeler,” that took center place in the news in Boston, and was interviewed by Eye Witness News for the banana peeler, and by Boston Public Television on the impact of automation on rural unemployment in Latin America and migration to the United States.

For 25 years he endeavored in the development of technology, start-up and troubleshooting for direct reduction plants worldwide.

Today he is the President of TECMEN, a consulting company dedicated to develop solutions material handling and process under extreme conditions that go from automated “taco” shell handling, to high temperature components applied to chemical and nuclear processes. He has produced multiple worldwide patents for different companies. He has traveled extensively, lately spending most of his time between Australia, Germany and South America.

He has been a TapRooT® instructor for seven years and is co-translator of the TapRooT® course materials into Spanish and Portuguese, and has frequently applied the TapRooT® method for innovation, project planning, and systematic failure and accident analysis.

He paints, writes poetry and wrote a play on the role of women in Mexican History.

Patrick began his industrial career working in the Operations group for a state-of-the-art pulp mill. Following a career in boiler operations he obtained a 2nd Class Power Engineer certificate while moving through various industries (Heavy Oil, Power Generation and Sour Gas). Shortly thereafter he joined the EH&S division of a major Oil and Gas Corporation, obtaining a CRSP designation. He also has a liberal arts degree in Biblical Studies.

As TapRooT® Lead for the Canadian Division with his company, Patrick was the “go to guy” for the integration of TapRooT®. In the fall of 2011 Patrick made a career change out of the EH&S world and into upstream technical sales in gas plants for Petro-Canada lubricants, a division of Suncor Energy. Currently he is enjoying learning a completely new career path and hopes to bring some of the Equifactor® process to bear on his role with Suncor.

Patrick has enjoyed a wide variety of activities including hiking, snowboarding, hang gliding, flying small aircraft and sailplanes, rock climbing, motorcycling, martial arts and most recently tinkering on his 30-year-old Porsche 911. When questioned about the dichotomy between “safety guy” and his love of higher risk activities he will respond with “risk management, its all about risk management”. His marvelous wife of 27 years will roll her eyes at that statement, given that their two teenagers are unfortunately now using similar rebuttals.

Lori Reineke-Avant

Lori Reineke-Avant is a married, suburban, working mother of two adult children and a 27-year resident of Atlanta, Georgia. One evening in the fall of 2009, while driving alone, Lori was involved in a traffic accident that resulted in fatal injuries to a pedestrian crossing the highway on a red light. Although witness testimonies confirmed the pedestrian had crossed the street illegally, three months later Lori was charged and arrested on a four count felony vehicular homicide charge that included texting while driving.  Her charges arose from a local statute that allowed for distracted driving charges in the absence of any law at that time that banned cell phone usage while driving. What had been a quiet suburban life for Lori turned in to a personal and public 2-year battle to prove her innocence of texting while driving during the accident. The story of her experience takes on many twists and turns, including false accusations of texting by a third party, a local political agenda to ban texting while driving and the lessons she has learned about the potential for criminal prosecution of accidents.

During the latter part of Brian Tink's career at Hydro One (starting in 1994), he specialized in health, safety and environmental matters. Before pursuing other interests, he held the title of Manager of Health, Safety & Environment Field Support for Hydro One Networks, Inc. This branch of Hydro One delivers electrical service to homes and businesses across the province of Ontario, Canada. Hydro One owns over 90,000 miles of Transmission and Distribution Lines, serves 1.3 million customers with 4,600 regular employees.

In this position, while reporting to the VP HSE, he was called to analyze and investigate safety incidents that occurred. He has facilitated or been part of the action plan development for numerous investigations, approximately 30-40 per year since 2000. The position involved working with and reviewing incident reports and action plans with other Line of Business VP’s and Directors. The lines of business included Construction, Lines, Station Maintenance and Forestry type work activities. He has also been called to lead or participate in investigations where a high level of scrutiny is expected for the results or there is significant need to identify areas of improvement. 

During his career, he had the opportunity to attend an extensive amount of training and seminars. The training resulted in becoming a Certified Registered Safety Professional in Canada and a Certified Utility Safety Administrator through the National Safety Council in the USA. Both of these certifications require safety management theoretical knowledge, testing and several years safety management experience.

Ken Turnbull is a Consultant and TapRooT® instructor with an impressive resume of worldwide experience. Ken specializes in developing proactive measurements for safety programs, coaching executives in demonstrating management commitment to safety, and doing root cause investigations.

He worked for Texaco for 32 years, and lead many accident and incident investigations as Texaco’s Corporate Safety manager. He used and taught the TapRooT® method for over eight years at Texaco and was instrumental in convincing executive management of Texaco the importance of reporting and determining the root causes of all incidents, including near misses.

Ken is a CSP, a former member of the NFPA 30 Committee, a former member of the Technical Steering Committee of the Center for Chemical Process Safety and a former Chairman of the API Safety and Fire Protection Sub-Committee. Ken has taught courses in the USA, Canada, Scotland, and Africa.

Bryan began his lean education while serving in the United States Air Force. Being part of a small, core team that was trained in tool & task kitting, cross training and flexible workspace design; Bryan was an instrumental part of the implementation team at three US Air Force locations. From there, Bryan has led numerous lean enterprise engagements and implementations at Raytheon Aircraft Company as a Program Administrator, Manufacturing Manager, Lead Six Sigma Black Belt in the Aftermarket Business Division, Manufacturing Operations and for numerous clients after leaving Raytheon. Bryan has dedicated the last 20 plus years to designing and improving processes, creating cultural change at all organizational levels and has a passion for continuous improvement.

Bryan has been an independent contractor since February of 2006 serving the role as change agent, project leader, workforce training developer and trainer within the Steel Fabrication, Mechanical Construction, Energy Exploration, Assisted Living and Non-Profit Industries. William Wiatrowski

William Wiatrowski has headed up the BLS Office of Compensation and Working Conditions since 2005. Prior to that he was Assistant Commissioner for the BLS Occupational Safety and Health Statistics Programs and held a number of staff and supervisory positions in the BLS wage and benefit programs.

The BLS compensation and workplace safety and health programs provide comprehensive information on employee wages and benefits and serve as the nation's public health surveillance systems for work-related fatal injuries and non-fatal injuries and illnesses.

Mr. Wiatrowski received a Bachelor of Arts degree in Economics and the History of Art from Yale University and a Masters of Business Administration degree from the George Washington University. He is the author of numerous articles on employee benefits, compensation, and occupational safety and health issues and speaks on these topics before a wide variety of audiences.
