Here’s just a few of the pictures I’ve collected of our highly qualified instructors teaching the 2-Day TapRooT® Incident Investigation and Root Cause Analysis Course, 3-Day TapRooT®/Equifactor® Equipment Troubleshooting and Root Cause Analysis Course, and 5-Day TapRooT® Advanced Root Cause Analysis Team Leader Course.

Arturo de la Garza - one of our Spanish speaking instructors with extensive industrial experience in Mexico.

Marco Flores - another of our Spanish speaking instructors. In addition to teaching TapRooT®, Marco, an MIT grad, consults with large industrial companies around the world to help them solve material handling problems.

Jim Whiting - Risk Management expert - is one of our Australia instructors.


Joel Haight - one of our instructors who also teaches at Penn State. Joel’s experience includes extensive oil industry experience (both upstream and downstream - in the US and overseas).

Ken Reed - SI’s lead for Equifactor®. Ken came to SI after a career in the Nuclear Navy (Master Chief).


Michele Lindsay - one of our Canadian instructors. Michele’s ergonomics and utility experience provide an excellent starting point for her teaching of the TapRooT® Root Cause Analysis System.

Sanjay Gandhi - our instructor from Kenya. Sanjay has extensive oil industry experience and safety experience consulting experience in Africa and Asia.


Steve Swarthout - extensive nuclear and military background gives Steve plenty of experience to relate in courses.


Kevin McManus - Kevin comes from a manufacturing and quality improvement background. He has been a senior Examiner for the Malcolm Baldrige National Quality Award.



Brian Locker - Another of our Canadian instructors. Brian’s previous experience includes utility and nuclear regulatory work. (OK - Brian isn’t teaching in the second picture. But I couldn’t resist putting in the “Kissed by a Camel” picture taken while Brian was teaching in Saudi Arabia.)


Dana Barclay - Dana’s background includes military aviation and work on the Columbia Shuttle disaster investigation.

Ken Scott - A career at MSHA preceded Ken’s work teaching root cause analysis for SI.
