Accidents still haunt US Navy sailors.
Here’s an article about a sailor lost overboard while lowering a small boat. He was never recovered.
This was a simple accident with fatal results.
Too bad these type of accidents happen over and over again. It seems the Navy just can’t learn. Perhaps someday they will implement advanced root cause analysis and expect CO’s to conduct effective incident investigations that focus on effective root cause analysis and corrective actions rather than blame.
