Horrible Death at Rice Factory Costs One Life and £140,000 Fine

SHP reported on a horrible accident at the Veetee Rice Ltd factory in the UK.

A worker entered a rice silo to clear a jam with the auger still energized. He sunk into the rice and his leg was trapped in the auger. It had to be amputated to free him. He died on his way to the hospital.

What physical safeguards do you have in place to keep people out of dangerous equipment?

Training isn’t enough. How many times have we seen “trained” employees take shortcuts to get the job done. The physical safeguard has to make it so difficult to bypass the rules that they employee decides that shortcuts just aren’t worth it.
