Second, is “Using FACT to Measure & Analyze Fatigue (Both Reactive and Proactive)”. If you’ve ever wondered how you can prove that fatigue was an issue in an accident, Bill has the answer and will share it at the Summit.

Root Cause Analysis Tips – It all starts with a good SnapCharT®
Wednesday, January 25th, 2012It is almost here – the 2012 Global TapRooT® Summit.
As you have probably heard, on the two days before the summit we have special pre-summit courses we only teach publicly at the summit (we can do any of them onsite). The course I teach is called the Advanced TapRooT® techniques course. In this course, we cover best practices for each of the steps of our seven-step process. A big focus of the course is identifying causal factors, and I will give you some insight into some of that in the root cause tips on February 15. Today I wanted to talk about something from the course that is extremely important. Take a look at this analogy: 
Have you ever wondered why sometimes you do an investigation, things get better, but there are still some of the same problems present – maybe they are not as frequent or serious, but they still occur. Why is that? A great deal of the time, I would say because you found some of the causal factors and their root causes, but you may have missed one. In order to make sure problems do not reoccur, you have to find ALL of the causal factors, and therefore, all of the root causes. And it all starts with a good SnapCharT®. You see, it is much easier to find the causal factors if your chart is very clear, is not missing important details, and is well worded. Most of the time you spend in your investigation should be on this front end of the process – the investment in collecting good information and assembling it on your chart will make finding the causal factors easier, and taking them through the Root Cause Tree® faster.

Assessing Organisational Risk Maturity – Malcolm Gresham
How do you assess the your organisation’s risk management maturity? It is no longer sufficient to implement a Risk Management Framework and hope for the best. Organisations need to assess whether their current level of risk management is appropriate. The Risk Maturity Model provides organisations with a valuable measure of the ongoing development of their ability to manage Risk.
Using the new Risk Management Standard ANSI Z690.2 : 2011 to Assess the Maturity of your Risk Management System – Jim Whiting
How mature is your organisation’s RMS ? This session discusses how to obtain assurance of the maturity, quality, adequacy of your RMS measured against conformity with the new ANSI Z690.2 Standard for RM Systems. This Standard provides specific audit criteria and guidelines for the principles, framework, and process required of a ‘Best Practice’ organisation. An audit process using ANSI Z690.2 criteria can be performed on both an Internal / First Party and even a External / Third Party basis. Either way the resulting benefits center on being able to demonstrate internally and/or externally that your organisation is managing risks in an adequate way. Hence the audit results can provide confidence and assurance to your own people as well as external stakeholders. The workshop will discuss how the evidence-seeking questions/ activities in such an audit can be constructed, delivered and their results analysed and reported.

Do we need reward or punishment… or both? How do different cultures interact successfully to work safely? What is the best environment for a person to work in physically? How does one know?
Listening to a radio show recently about people trying to get out of debt, the host said this, “it is not the math that got them in the situation it is the behavior; that is why changing the behavior is the first step.” It was in reference to people who wanted to know why the had to pay off small debts first and not the large credit cards with high interest.
Point being, that the more one practices a behavior, the higher the probability that the behavior becomes habit. Providing a better environment with the right tools increases the ability to perform the behavior. It is with this in mind that the sessions below were put together:

Investigation, Troubleshooting, & Root Cause Analysis Track at the 2012 Global TapRooT® Summit
Tuesday, January 10th, 2012We’re counting down to the 2012 Global TapRooT® Summit in Las Vegas on February 29 to March 2. Don’t miss the opportunity to be part of the growing improvement trend by attending the premier root cause analysis event.
One of the exciting tracks this year is the Investigation, Troubleshooting, & Root Cause Analysis Track. We have lots of outstanding sessions included in this track. You can see the full schedule by clicking this link. Here is a quick run-down of the sessions:
1. TapRooT® Implementation, Investigations, and Process Improvements. This session will include talks by 3 different speakers, describing how they have been successful implementing TapRooT® at their facilities. Janis Chartier (PPL Electric), Mike Ezzell (Price Gregory Construction), and Mark Eliopulos (Prevention Opportunities) will give specific examples of how TapRooT® has been used to effectively investigate problems at their facilities. There’s nothing better than actual examples to give you great ideas for improvements at your own facilities.
2. What is “Culture” & How Do You Identify & Solve Culture Problems Using TapRooT®? Brian W. Tink, Brian A. Tink (yep, father and son), Richard Mesker and Clare Solomon will discuss company culture and how that directly relates to the TapRooT® process, with direct relationships to the Root Cause Tree®. How do you actually measure and define your company’s culture? How do you communicate a message to help change a culture?Think you understand “culture”? Check out this session for a terrific explanation of how culture and TapRooT® fit together.
3. Troubleshooting Equipment Problems as Part of a TapRooT® Investigation. Many investigations end up with an equipment failure as one or more of the Causal Factors. How do these failures fit in with the general investigation process? I’ll be discussing how machinery failures, while often a piece of the puzzle, are often more a symptom than a cause.
4. CAPA in Quality: The Strong and the Weak. Where are those quality managers out there? Karen Migliaccio will host a lively on discussion on how quality issues can be investigated using TapRooT®.
5. TapRooT® Users Share Root Cause Best Practices. Linda Unger and Michele Lindsay will be facilitating an interactive session for all users. Come and give your ideas, or just sit back and absorb the ideas from others. Always a great session!
6. The 7 Secrets of Incident Investigation & Root Cause Analysis. While many companies are reaping the benefits of sustained, productive investigations, some companies still struggle. Mark Paradies’ session will share secrets and lessons learned about what makes investigations fail, and more importantly, ways to improve your investigation performance.
7. Investigation Process Best Practices. This is a 2 for 1!!
- Does your investigation process seem to take too long? Don’t blame TapRooT®! I’ll be hosting a discussion on how to optimize your investigation process.
- Karen Gallogly will then be presenting an overview of the IEEE Nuclear Root Cause Analysis Standard.
8. The Day 29 Miners Died: The UBB Mine Explosion. When a tragedy strikes, it is imperative that we learn from the mistakes that were made. Ken Scott (former MSHA inspector) will lead a discussion on the explosion that took the lives of 29 miners.

NEW 2012 Global TapRooT® Summit Session Added to Safety, Process Safety and Risk Management Track
Monday, January 9th, 2012A new 30-minute talk has been added to the Safety, Process Safety and Risk Management Track session, “Risk Management & Safety,” (Thursday, March 1 at 10:50 a.m.). Australian risk management expert and TapRooT® Instructor, Jim Whiting, will present “Using the New Risk Management Standard ANSI Z690.2 : 2011 to Assess the Maturity of your Risk Management System.”
How mature is your organization’s RMS? This 30-minute workshop session focuses on one part of the 2-day pre-Summit course, Risk Management Best Practices, i.e. how to obtain assurance of the maturity, quality, adequacy of your RMS measured against conformity with the new ANSI Z690.2 Standard for RM Systems. This Standard provides specific audit criteria and guidelines for the principles, framework, and process required of a ‘Best Practice’ organization. An audit process using ANSI Z690.2 criteria can be performed on both an Internal / First Party and even a External / Third Party basis. Either way the resulting benefits center on being able to demonstrate internally and/or externally that your organization is managing risks in an adequate way. Hence, the audit results can provide confidence and assurance to your own people as well as external stakeholders. The workshop will discuss how the evidence-seeking questions/ activities in such an audit can be constructed, delivered and their results analyzed and reported.

9. Our competition is attending. We’ll just wait and

8. It conflicts with my karaoke lesson.
7. There’s too much to do in Las Vegas. I get confused with too many choices.
6. People learn too much at those Summits. It might make my head explode.
5. I like freezing cold weather. I’ve heard it gets above zero in Vegas in the winter.
4. I haven’t had the shots required to travel to Las Vegas.
3. I’ve heard they have a web site and I don’t like spiders!
