Here is the story:

Pretty scary – I hope these individuals recover and the organization determines what happened, why it happened, and develops good corrective actions.  That may sound obvious, but sadly, many organizations do not do good investigations – not because they don’t want to, but because they do not have a good system, do not ask the right questions, and do not apply the right knowledge and best practices to their corrections.  People also have a tendency to let the investigation lead them within their comfort zone and to start fixing things before they really know what is broken.
