If you have flown recently…. who could miss the sign that states do not enter here? Next to the sign is a blocked corridor and a guard… right? Very few airports that I visit have a guarded corridor. So for those who have attended a TapRooT® Root Cause Analysis course, the first issue you would identify is whether a sign is an effective control.

In this case, a passerby notified security about the breach, so the sign did not work. If you truly want to reduce the hazard of uncontrolled entry to an area, signs are definitely not enough.
