Are You Writing the Same Corrective Actions?

April 17th, 2017 by

Repeating the same corrective actions over and over again defeats the purpose of a quality root cause analysis investigation. If you spend the time investigating and digging deeper to find the REAL root cause, you should write the most effective corrective actions you can to ensure it was all worth the resources put into it. Instructor & Equifactor® and TapRooT® Expert, Ken Reed, talks about corrective actions and how to make them new and effective for each root cause.

Take a TapRooT® Root Cause Analysis course today to learn our effective and efficient RCA methodology.

System Improvements/TapRooT® Television Segment Scheduled for CNBC

September 6th, 2012 by

Here is the press release for the upcoming segment:

Boca Raton, FL (TBD) — Multi-Media Productions (USA), Inc. is pleased to announce that System Improvements, Inc./TapRooT® will be airing on 21st Century Business on CNBC (as paid programming).

Safety incidents, equipment failures, quality issues, sentinel events, and production upsets cost business owners billions of dollars a year. Many times these incidents are ongoing and repetitive. Companies try different “fixes” but they just don’t work. TapRooT® is a systematic process for finding the real root causes of problems so that the problem can be eliminated entirely.

TapRooT® is a book, a system, training, and softwarefor root cause analysis of problems. The TapRooT® System helps people solve problems by finding and fixing root causes that, when corrected, stop problems from happening over and over again.

TapRooT® is used by leading companies around the world to investigate and fix the root causes of major accidents, everyday incidents, minor near-misses, quality issues, human errors, maintenance problems, medical mistakes, sentinel events, productivity issues, manufacturing mistakes, production delays, and environmental releases – in other words, almost every type of problem you can think of.

The TapRooT® System is based on advanced, proven human performance and equipment reliability technology – theory and models of human performance and equipment performance from experts including Heinz Bloch, Dr. Jens Rasmussen, Mark Paradies, Dr. James Reason, Linda Unger, Dr. Charles Hopkins, Andrew Marquardt, Jerry Lederer, Larry Minnick, Ed Skompski, Smoke Price, Dr. Chris Wickens, James Haddon, and many more. This knowledge was built into the TapRooT® System so that every person investigating problems can take advantage of the knowledge of experts without having to spend years studying human and equipment performance.

TapRooT®’s ground breaking research and development and continuing improvement efforts are the reasons that TapRooT® is so highly praised by users and is chosen by so many industry leaders as their standard root cause analysis tool to analyze mission critical problems.

However, TapRooT® isn’t just for complex problems or high risk operations. The common sense approach and straightforward nature of the system is why people use it to analyze every day problems.

JL Haber, Vice President of Programming at Multi Media Productions, added, “With a unique approach to root cause analysis, TapRooT® was a natural fit for our show. We are excited to have them as a guest on our program.”

Global Root Cause Analysis eNewsletter – More Valuable Resources!

July 9th, 2011 by

TapRooT® Friends & Experts:

This week we have added some content to the TapRooT® Friends & Experts (Root Cause Analysis) eNewsletter that we think you will find of value.

First, we’ve added a few past articles to the Root Cause Analysis tips column.
Our instructors write about topics that you have asked at our courses.  We know you’re busy, so we want to give you every opportunity to benefit from this valuable information and ongoing training by highlighting these past topics in the weekly eNewsletter before they get buried at the bottom of the blog.

Second, we’re adding reader feedback to some of our columns.
This week you will notice it in the Career Development column and in the TapRooT® Mail column.  Our subscribers have very thought-provoking ideas and we encourage you to join the discussion and share ideas.

Finally, we love multi-media!
Our video links are easy to miss among all the information we send you in the eNewsletter, so this week I’ve highlighted them with a video icon.

Let us know what you think about these improvements when you get your next edition on Tuesday, July 12.

And thank you for being a valued subscriber!  (If you are not yet subscribed to the best global root cause analysis eNewsletter available, please send a request to me, Barbara, at editor(at)taproot.com with “please subscribe me to the eNewsletter” in the subject line.  The eNewsletter delivers the latest news and information from the Root Cause Analysis blog to your inbox every Tuesday.)
