Technically Speaking – When is the PEN mightier than the KEYBOARD?

When System Improvements lets me out of my cage (the cage where all the code-monkeys bang on typewriters until the latest release of the TapRooT® Software is complete), I often find myself in a TapRooT® course. Next week, for example, I will be teaching the TapRooT® 2 Day Course in Portland, Oregon using the  TapRooT® VI software!

When mingling with the users of the TapRooT® Software at a course, I often get asked the question, “When should you use the software during your investigation and when should you perform the process without the software?”

I think people typically expect me to say, “ALWAYS use the Software!”, because, well, I’m fond of job security.  

But in reality, I believe The TapRooT® Software is a valuable tool in performing an investigation, but like any other tool, you should only use it to accomplish the appropriate task. Sometimes the PEN is mightier than the keyboard.

You would not use a chainsaw to unclog your toilet, would you?
If you answered, “No of course not!”, then keep reading.
If you answered, “Yes”, then maybe you should stop reading and go call a plumber.

So “When”, you ask, “do you use the software and when do you not?”
My answer, “Stop interrupting, I was just about to tell you!”

GETTING STARTED
When you have a report of an incident and are just getting started, this is a great time to use the software. It is pretty simple to do:

• Enter 5 pieces of required information (Title, Description, Location, Classification, Date)

Pretty simple right?  Now you are in business, because the 7 Steps (or 5 Phases in Version VI) are going to guide you through the rest of the process.

The first step in that process is to create a new Spring SnapCharT® and add a few simple things to get your investigation started:

• Define your Incident (Circle Shape)
• Add the Events you know occurred (Rectangle Shape)
• Add QUESTIONS you want to ask when gathering evidence (Oval Shape)

After you do this, you are ready to go interview and gather evidence to answer those questions.

INTERVIEWING AND EVIDENCE GATHERING
Can you type with the quickness of a stenographer, while making eye contact with who you are talking to and, oh yeah, remember to LISTEN?

Then hey, bring along the laptop, because you are a superhero (The Magnificent Multi-Tasker?).

If not, maybe a simple notepad (or tablet) and a print-out of your Planning SnapCharT® will be sufficient.   Although, TapRooT® VI DOES work on a tablet if you are feeling adventurous and mobile.

SNAP TO IT!
Once you have all your interviews done and your evidence collected, it is time to map out your sequence of events and build your SnapCharT®.

I actually recommend 2 different approaches at this stage. If you are doing an investigation by yourself, then by all means open up the software.

However, if you are part of an investigation TEAM, then you need to turn off that computer and projector.  You simply do not want an investigation team staring at a projector screen (or worse, a small laptop screen) as you assemble a SnapCharT® in the software.

Janet from EH&S will inevitably fall asleep and drool all over the conference table, Bobby from manufacturing is going to keep asking, “What does that button do?”, and Rick (who is supposed to be YOUR ASSISTANT) is going to keep pointing out your grammatical errors.  What is THERE, errr…. THEIR, problem?

So, bust out the sticky notes or a dry erase board and work it through as a team, ON YOUR FEET. You’ll be surprised at how engaging it can be. You can always take a picture of the results and easily assemble the SnapCharT® in the software later.

I NEED SOME ‘ME’ TIME
After you have a clear understanding of the sequence of events, and have added all the detail your conference room wallpaper can support in sticky notes, it is time for you to make like Han and go Solo.

Tell your team you will schedule another meeting to discuss Root Causes and Corrective Actions at a later date, after you have had some time to work through the findings.

Explain to them it is not them, it’s you. You just need some alone time to analyze the incident (and your feelings).

Are they gone? Okay, now it is time to fire up the software and:
• Recreate your SnapCharT® (while getting a soothing massage)
• Isolate your causal factors on your  SnapCharT® (during an avocado facial scrub)
• Run each causal factor through the Root Cause Tree® (while listening to Enya near a fountain)

Did you get to the Root Causes (and figure out why you lash out sometimes)? Well then…

WE’RE GETTING THE BAND BACK TOGETHER!!
Call up Janet, Bobby and Rick because it is time to reunite (it will feel so good)!

Review the Root Causes you found during your hiatus, but do not dare open up those Root Cause Trees® in the software! You are not here to revisit the past. You are here to move forward and make new catchy Corrective Actions that your audience (Management) is going to love to hear.

Feel free to use the software to consult the Corrective Action Helper® often. Work on that setlist until you have it perfected because the big show is coming up…

THE BIG SHOW
As you walk into the conference room, Janet, Bobby and Rick behind you, you know you have brought the goods to wow your audience (Management).

But what didn’t you bring? Your Root Cause Trees® or your FULL SnapCharT®s.

All you need to pull off the big show is your Presentation SnapCharT® (which you have pruned in the software to show only the information relevant to presentation), your Root Causes, and your Corrective Actions.

Technically Speaking – A Sneak Peek at TapRooT® VI (A Closer Look at SnapCharT®)

We are nearing the initial release for TapRooT® VI and excited to announce that the first public 2 Day Course with the new software will be taught by software guru Dan Verlinde in Portland, Oregon on April 18th. Join us if you can! If you can’t make it though, don’t worry. I have more to share with you now.

The past couple of Sneak Peek blog articles have highlighted many of the exciting new enhancements found throughout TapRooT® VI but let’s take an even CLOSER look at one of the most used features of the software:  SnapCharT®.

Creating a SnapCharT® is a critical step within the TapRooT® process and TapRooT® VI is aimed at providing a LOT of enhancements to make this easier to use and also more visually appealing. Take a look:

Some things you’ve probably noticed right away:

On the left hand side of the SnapCharT® menu, there are new TABS that allow you to do more within the SnapCharT®.

  • The Shapes Tab contains your standard SnapCharT® Shapes and allows you to add Conditions, Events, Incidents, Page Breaks and Text to your chart.
  • The Optional Techniques Tab allows you to create an Equifactor®, Change Analysis, Safeguards Analysis and/or CHAP DIRECTLY from your SnapCharT®. You no longer have to create your Optional Techniques independently of the SnapCharT®. While using any Optional Technique, you can identify and create new Conditions or Events. Then, use the next tab (Drop Zone) to immediately add them to your chart.
  • As mentioned above, the Drop Zone Tab is an area where shapes created using the Optional Techniques will appear. From here you can choose whether or not to add them to your SnapCharT®.
  • The Versions Tab will display a list of SnapCharT® COPIES. At any point in the process you can make a COPY of a SnapCharT® to keep for your reference later on. You can make as many copies as you want!

Some things you may NOT have noticed that have also been incorporated into the newly designed SnapCharT®:

  • Auto save. No more worrying about having to click the save button! SnapCharT® will save your changes every 1 minute OR at any time IF exiting the SnapCharT®.
  • Typing in your Condition, Events or Incident Shapes is easier than ever. Simply add the shape of your choice to the SnapCharT® and start typing. You don’t have to add your shape, click in the text field at the top of SnapCharT® and enter your text here anymore.
  • Take pictures of the chart (or part of the chart) using the SnapCap Image Capture feature and they are saved directly to a SnapCap folder that can be found in your Attachments section.
  • With the simple click of a button, you can change an Event to a Condition or vice versa. No more adding, moving or deleting shapes just because you want to change it. Keep your text in place and simply click the “Change” button.
  • EVERY USER can assign their own custom colors, and font types, sizes, bold/italics/underline settings for EACH SnapCharT® shape. And once you indicate these preferences, they will carry over to every SnapCharT® you do.

Technically Speaking – A Sneak Peek at TapRooT® VI (Q and A Edition)

I recently attended a 5 Day Course (it was awesome!) so I had a chance to get feedback directly from people in the field using the TapRooT® Version 5 Software. I received lots of great questions and comments so I thought I would share some of the questions (and answers) with you. You may notice a theme here…

Q: On the SnapCharT® in Version 5, if I decide I need to change an Event to a Condition or a Condition to an Event, can I do this without having to add a new shape and delete the old one?

A: No. BUT, you can in TapRooT® VI!
With the simple click of a button, you can change an Event to a Condition or vice versa. No more adding, moving or deleting shapes just because you want to change it. Keep your text in place and simply click the “Change” button.

Q: Can I apply default colors, and fonts, sizes, etc. to my Events, Conditions and Incidents in Version 5?

A: No. BUT, you can in TapRooT® VI!
EVERY USER can assign their own custom colors, and font types, sizes, bold/italics/underline settings for EACH SnapCharT® shape. And once you indicate these preferences, they will carry over to every SnapCharT® you do.

Q: When I’m using any of the Optional Techniques in Version 5 (like Change Analysis, CHAP, Equifactor® or Safeguards Analysis) and I identify a Condition or Event that needs to be added to my SnapCharT®, can I do this without having to leave the Optional Technique, open my SnapCharT® and manually add the shape and text?

A: No. BUT, you can in TapRooT® VI!
The new and improved Optional Techniques allow you to immediately take your information and click a button that adds your Condition or Event to the SnapCharT®. It is added to a “Drop Zone” area and when you return to the SnapCharT®, your new Condition or Event is waiting there for you to drag and drop it onto the chart.

Q: When I’m using the Root Cause Tree® and I want to remember specifically which questions I answered “Yes” to, can I indicate this in the Version 5 Software or do I manually have to add a comment?

A: No. BUT, you can in TapRooT® VI!
How many times have you gone back to an Investigation and wondered, “Why exactly did I decide to choose this Root Cause?” Wonder no more! TapRooT® VI still has the capability of saving Root Cause Tree® Analysis Comments, but it also allows you to select “Yes” or “No” to any question from the Root Cause Tree® Dictionary with the click of a button.

Time to Upgrade Your Web Enterprise Software to 5.3

Microsoft® recently released an update that has caused some previously supported components to produce the error:

This application has requested the Runtime to terminate in an unusual way. Please contact the applications’s support team for more information.

If you are operating on an older version of the TapRooT® Web Enterprise Software (Version 5.2.3 or older) then you are at risk of seeing this error when you upgrade your network to the latest Windows Updates.

The error does not exist in any of the latest versions of TapRooT® including:

Version 5.3.0 (released in March 2014)

Version 5.3.1  (released in August 2014)

We recommend upgrading to the latest version as soon as possible.

TapRooT® Software Version 5.3.2 Release Announcement

We are very pleased to announce the release of our Version 5.3.2  TapRooT® Single User Software.

The majority of the enhancements in the Version 5.3.2 TapRooT® Software were to repair minor bugs or known issues.  A detailed list of these enhancements is found below.

TAPROOT® 5.3.2 ENHANCEMENTS

  • Investigation Reports display issues were resolved for those who had non-standard SQL Collation installations.
  • Custom Details fields are now visible on the display list, even when they are inactive.
  • Custom Details Currency fields are displaying the label and currency symbol properly in all cases.
  • Corrective Action Priority field is restored.
  • Corrective Action Editor now validates and prompts if you want to save your work when selecting CLOSE.
  • Root Cause Tree® validates and displays all changes when switching between front and back page.
  • Root Cause Tree® displays all Red X’s in print mode.
  • Root Cause Tree® documents can be deleted from the 7 Step Process page.

Version 5.3.1 Software Release Announcement

We are very pleased to announce the release of our Version 5.3.1 TapRooT® Software in both Web Enterprise and Single User versions.

The majority of the enhancements in the Version 5.3.1 TapRooT® Software were designed to add functionality to the user and make the navigation of the software more intuitive. A detailed list of these enhancements is found below.

TAPROOT® 5.3.1 ENHANCEMENTS
o In Single User, you can now create and edit Locations, Classifications, Custom Details Fields and Attachment Folders from the Investigation Editor. You no longer need to go to the Admin menu to accomplish these task.
o When Saving a SnapCharT® as a different season (Spring, Summer, Autumn or Winter) you no longer have to rename the SnapCharT®. The selection of a new season creates a unique name automatically.
o The upcoming new Root Cause of ‘Language’ has been introduced on the Root Cause Tree®
o Under Techniques>Manage Documents, new installations will have a SnapCharT® template to use which helps remind new TapRooT® users of the 15 questions.
o Assorted Bug fixes

This comes less than 2 months after we released Version 5.3.0 with the following enhancements:

TapRooT® System Software Version 5.2.1 Single User Release Notes

The TapRooT® Technical Support team is excited to announce our new Version 5.2.1 of the TapRooT® System Software.  We’ve heard from you and have implemented many new features you’ve suggested.

5.1.5.4 Hotfix is released!

The TapRooT® Technical Support team is happy to announce the release of the Version 5.1.5.4 Hotfix for the Single User and Web Enterprise Software.   This release includes the following updates:

  • The SnapChartT® Date/Time box accepts values between 12:00 and 12:59 AM when the software is configured in 12 hour mode.
  • Near Root Causes with Corrective Actions assigned to them are now appearing on the Investigation Report and Corrective Action Report.
  • After chart has been saved, the date field appears on the SnapCharT® objects.
  • UserImport.xls correctly imports user First Name and Last Name fields.
  • Corrective Actions are now showing up on the Corrective Action Summary By Causal Factor Report when there is no Corrective Action Number set.
  • There is no Javascript error when opening Root Cause Trees.

TapRooT® Software Version 5.1.5 Upgrade

We’re pleased to announce our latest upgrade to our TapRooT® Software, which has new and updated features that make investigation reporting easier than ever.

Updates

Foreign Language – Now your system administrator can set your default language for the Root Cause Tree Dictionary and Corrective Action Helper to French, German, or Spanish.

Search Function – We have completely redesigned the software’s home page, search page, and documents open page to put powerful search tools at your fingertips. You’ll find the exact document you need, making your investigation reporting quick and simple.

Corrective Action Table – The Investigation, Audit, and Corrective Action reports are cleaner and sharper looking and come equipped with the Corrective Action Table featured in the TapRooT® Book. This visual aid will help each person understand the corrective actions for which he or she is responsible and get them done on time.

Window Resizing (Single User) – Now you can resize and maximize any window in the Single User Version, including the SnapCharT® Overview window. You’ll have more space to work on that SnapCharT®, making your reporting simpler than ever.

Redesign – The TapRooT® Software’s borders are visually leaner and trimmer than in previous versions, giving you more workable space.

Root Cause Tree Creation – The software only allows you to create a Root Cause Tree® when you save an Autumn SnapCharT® with causal factors. You’ll never again accidentally create a new Root Cause Tree® each time you create the next season’s SnapCharT®.

Multiple Reports – You can now run multiple reports at once from the Investigation Reports page, making reporting fast and efficient.

PDF Software Tutorial – Now you can download a PDF copy of the software tutorial from the Help Menu. You’ll get your questions answered quicker than ever.

Date Selection Boxes – We have infused the date selection boxes with new technology, making them easier for you to use than before.

System Configurations – Software administrators can use WebConfig.exe to conveniently change timeout and database settings. The default timeout setting is now much higher. Also, when administrators import multiple users with the Import tool they can set User Group and Foreign Language for these users.

Bug Fixes

– Saving the Root Cause Tree page has been completely overhauled. We believe this will resolve the issues that occasionally resulted from saving on this page.

– SnapCharT® now scrolls to the right more easily when dragging a shape.

– Copy and Paste functions have been restored to Analysis Comments.

– SnapCharT® SAVE AS button now recognizes changes since the previous save.

– All reports now paginate correctly.

– Investigation Editor Custom Details fields now wrap text properly.

– When signing off a Corrective Action, the date no longer defaults or incorrectly saves the year.

– In Enterprise version, the Temp directory is emptied to prevent issues from it becoming too full.

Root Cause Analysis Tip: What Can Go Inside the Event Shape when Mapping out your SnapCharT®?

Whether doing it by hand or in our TapRooT® Software, what can go into the Rectangles that we call Events (Who did whats or what did whats that occurred during the timeline that you are investigating)?

Actions by the Operator, Mechanic, Manager, Vendor, Supplier, Contractor, Technician, Customer Service Rep, Engineer, Designer, Nurse, Doctor … as you can see the list is unlimited but understanding the who (we use job titles only) helps us to see if the who was setup for success prior or during the action he/she performed.

Whats:

Caution ( … this may not be what you expected or have been doing)

Equipment Actions: Relay opened when energized, Butterfly valve stuck shut, I.V. bag port become blocked with debris, fuel gravity fed into container through piping …

Hint: If working with equipment, pull up the equipment and system functional diagram up immediately to help you map out the Events.

Chemical Process Actions: Catalysts heated up, hot mix heated up …

Transactional Process: Purchased order received by customer service, SAP sent late warning to warranty …

Hint: Yes, you can follow a piece a paper, hazardous material shipment.. that is handed off from person to person just like you would a person.

Root Cause Analysis Tip: Add Color to your SnapCharT® in v5 Easily

Users in the new v5 application can easily add color to every shape on their SnapCharT® by following these simple steps:
  1. Open the investigation or audit you are working on by highlighting it in the My Documents section of the User Landing Page and selecting OPEN.

  2. Through the 7-Step Process Menu select OPEN for the investigation you are working on under Manage Documents by any SnapCharT® step. Then selecting NEW on the Associated Documents Screen.

  3. Add a shape to the chart by selecting the proper shape under Create Chart and then double clicking in the editor.
  4. With the shape selected, select the Shape Color drop-down and select the desired color, and select OK.

  5. While the shape is selected, click inside the shape once the color is added (this ensures the shape is selected).
  6. Simply hit CTRL-V and you will be able to duplicate that shape with colors. Do this multiple times to add as many shapes as you like.
  7. Repeat this process for other shapes as added.
  8. Move shapes where needed and add text using the Text editor at the top center.
