Aviation Accident Investigation: Drunk? Disoriented? Fatigued? Un-Trained? Or All-of-the-Above?

Reuters reported that An Aeroflot 373 veers out of control and crashes while passing through a cloud at night.

The aviation investigators find the cause of the crash … TRAINING.

But they also say the crew “lost orientation.”

They also mention alcohol in the pilot’s bloodstream and excessive fatigue but say that these weren’t the main causes …

But Alexei Morozov, head of the investigating commission, emphasized:

“I repeat again: the direct cause of the accident
was the loss of orientation in space.”

And that’s the problem when you must arrive at a single cause.
