3-Day Equifactor® Equipment Troubleshooting & TapRooT® Root Cause Analysis

Repeat equipment issues? Investigations keep finding that human errors drive most equipment failures and you don’t know what to do about it?  Firing people isn’t an option: we can’t find anyone to replace them. Blame never fixes problems anyways. If you want to remove the real root causes of equipment failure and build true reliability into your systems, this is the right course for you!

This course is for accident investigators, maintenance teams, operators, quality specialists, and managers who need to get operations back up and running yesterday AND prevent it from happening again. We focus on the techniques needed to investigate low-to-medium risk incidents and precursor incidents. Our 5-step investigation process emphasizes TapRooT® makes investigations efficient yet effective. Training focuses on

  • Understanding what happened and troubleshooting using tools like Equifactor®
  • Identifying and analyzing Causal Factors using the Root Cause Tree® and Root Cause Tree® Dictionary and
  • Developing effective fixes using the Corrective Action Helper® Guide.

Equifactor® Techniques are based on the work of the late equipment reliability expert Heinz Bloch, and the TapRooT® Root Cause Analysis system was developed by Mark Paradies, founder of system improvements.
