“I just forgot”

Simple errors, simple solutions?  This driver just forgot to lower his bed before moving to the next work site.  Luckily, no one was hurt.  Read about it here.

How would you prevent this error?  Realistically, we’re probably not ready to answer this question.  Without a proper root cause analysis, it’s too easy to jump to conclusions.  I’ll bet the drivers stuck in the traffic jam on I-84 were ready to apply their own corrective actions, and I’ll also bet they all involved discipline in one form or another!
