There are four new worksheets to help you be more thorough in identifying Causal Factors while also saving time. How can people trained to use TapRooT® learn to use these worksheets? By participating in a 90-minute webinar that explains the worksheets and gives you practice using them to identify Causal Factors.

The four worksheets are for different kinds of investigations:

  • Quality
  • Equipment Failure
  • Patient Safety

This webinar will show you how to use these worksheets to more completely and effectively define Causal Factors in less time.

This webinar will cover:

  • The contents of the four worksheets
  • How to use a worksheet to identify Causal Factors
  • Walkthrough an example using a worksheet
  • Walkthrough an example using the worksheet build into the TapRooT® ® Software

Meet the instructors…

Tim Diggs is a TapRooT® Strategic Advisor and Instructor and has 24 years of experience in Logistics, Transportation, and Continuous Improvement initiatives. Tim has a BA in Economics and Management from the University of Tennessee.

Alex Paradies is a TapRooT® Strategic Advisor and Instructor and has seven years of manufacturing management and lean implementation leadership and was an internal ISO 9001 Auditor. He has a BS in Material Science and Engineering with a minor in Business Administration from the University of Tennessee.

Grading Investigations – May 6

Would you like a consistent way to evaluate investigations and coach investigators to improve their skills? To provide management with better incident investigations, root cause analysis, and corrective actions? To measure the improvement of investigations over time? If this sounds interesting, then this course is for you!

System Improvements has developed, tested, and improved a grading system for incident investigations. It is based on the TapRooT® 7-Step Process and the science of evaluation and feedback.

The system can be used directly in the TapRooT® VI Software (version 6.8 and beyond) or in an Excel spreadsheet.

This 90-minute webinar, led by Alex Paradies and Mark Paradies, will:

  • Explain how to use the grading sheets/software
  • Review the provided grading criteria
  • Provide an example of the use of the grading system
  • Allow you to try the grading system and ask questions
  • Discuss the use of the grading system by TapRoot® Users and the results they have achieved

Participants will learn to use the grading tools to evaluate and improve investigation. Participants will also consider ways to implement the grading process as part of their incident investigation process and policy.

Mark Paradies

Mark Paradies is President of System Improvements and co-developer of the TapRooT® System. He has 40 years of experience in high-reliability organizations, process safety, incident investigation, process improvement, and root cause analysis. His career started as a leader in Admiral Rickover’s Nuclear Navy (where he earned Engineer Qualification). He also worked for Du Pont in the areas of human factors, process safety, and performance improvement management before he started System Improvements in 1988. He has a BS in Electrical Engineering and an MS in Nuclear Engineering (with an emphasis on human factors), both from the University of Illinois.
