Man falls from a pallet on a forklift?

Why would someone be on a pallet on a forklift?!

I know what you are thinking; I’m naive.  Actually, I’m not, I just said that to make a point – I’ve seen behavior like this myself, more times than I can count.  There is not a lot of information in this article, but some initial questions that come to my mind are – what is the proper way to do this job?; what kind of equipment is needed?; who was in charge?; is this normal and are there other risky processes taking place in this workplace?

Lots of questions, but again, what I do know is that this did not have to happen.
