Monday Accident & Lessons Learned: Fatal Accident at James Street Station, Liverpool

The UK Rail Accident Investigation Branch posted a new report on a recent accident. Here’s the summary by the UK RAIB and a link to the full report.

The RAIB concluded its accident investigation in October 2012 and provided embargoed copies of its report to those involved, to enable them to start acting on its safety recommendations immediately.

The Crown Prosecution Service asked the RAIB to delay publication of its report until after the trial of the guard involved in the accident, so the RAIB did not make it available on its website at that time.

The RAIB released the report after the guard’s trial concluded in November 2012. The report makes three recommendations.
