Open Manhole “Causes” a Fatality

I am passing it along in the interest of safety education and fatality prevention so that others may learn and avoid a similar fatality.
After viewing the PowerPoint I was left wondering … Did they really understand the causes of this fatality? Also, did they fix everything they needed to fix? In other words, did they learn as much as they should have from this fatality? The PowerPont didn’t convince me.
The PowerPoint did get me thinking about how much the TapRooT® Techniques (not used in this investigation) can help investigators discover the root causes of accidents and present what happened, why it happened, and what can be done to prevent a similar incident from happening again in the future.
