Proactive Analysis Using the TapRooT® System Methodology & RCA Tools

TapRooT® System

Most organizations begin using the TapRooT® System for reactive investigations, which is great because each investigation will lead to continuous improvement.

After an incident occurs, your team uses the TapRooT® System SnapCharT® to help gather and organize the evidence they collect to build a visual representation of the incident. That helps everyone gain a powerful understanding of what truly happened.

After the team finalizes their SnapCharT®, they will analyze the incident and identify the human mistakes or equipment failures that directly caused the incident or made the consequences worse. We call those mistakes and failures Causal Factors. These Causal Factors are the mistakes and failures that directly caused the incident.

Once the Causal Factors are identified, your team uses TapRooT® RCA human performance and equipment expertise embedded in the TapRooT® Root Cause Tree® and Dictionary.

TapRooT® System

Your team will answer a series of expert questions using the evidence they uncovered during their investigation. The evidence will help them answer the questions which select and eliminate cause categories until the root causes of the mistakes and failures are found. These tools take you beyond your current knowledge and standardize your approach to problem-solving. It’s an evidence-driven RCA methodology. here is no guesswork, bias, or blame.

Now with all the root causes of the incident in front of your team, they use the Corrective Action Helper® to build more impactful corrective actions than retraining, disciplining, or rewriting policy and procedure. The corrective actions will get away from fixing people and will strengthen your systems so your workers are better supported and therefore perform better.

Once you find the root causes of the significant issues, you’ll use the Corrective Action Helper® to fix the problems BEFORE they cause an actual incident. Your team will proactively improve systems to lessen the risk of mistakes and failures.
