Many of the problems we encounter in life are the ones that we personally make. Nowhere is this fact more prevalent than at work, where our production assets perform according to our maintenance and operating choices. This session uses TapRooT® to analyze two of history’s most significant process safety failures, (the 1984 toxic chemical release in Bhopal, India, and the 1989 chemical plant explosion in Pasadena, Texas USA). This analysis reveals how our biggest problems often come about as we try to solve the little ones.

Kenneth Bloch is a Process Safety Supervisor in the Texas Gulf Coast area with over 30 years of Hydrocarbon Processing Industry experience. He regularly speaks at professional conferences to share experiences needed for companies to achieve their Process Safety Management (PSM) goals. In 2016 Ken published Rethinking Bhopal (ISBN #9780128037782), which explains the sequence of events behind history’s worst industrial disaster based on updated information sources.
