Shared Responsibility for Oil Spill

I love to read the verbiage used in articles; in this one my favorite phrase is “litany of blame.”

In this case several parties made errors and the discussion centers around who is “most” at fault.  Does that really matter, or should all the problems be analyzed and corrected?  OK, I’ll get off my soapbox now.

When you have an incident involving several errors and different departments, organizations, etc. it can be rather difficult identifying all the root causes; after all, we are talking about human performance.  The best way to investigate is by using a systematic approach with an expert system based on years of research; that would be TapRooT®.
