Subway Near Miss Under Investigation

The UK Rail Accident Investigation Branch has released an initial report of a near miss involving a London Subway.  You can read the summary here.

We can’t tell what actually happened yet, so it is too early to make too many judgments.  However, it appears an automatic interlock brought the train to a halt after the system detected an unsafe condition.  Looks like some safeguards were in place and worked. Now the investigation will focus on the root causes of why the interlocks were forced to engage.
