What's the Root Cause of Our Biggest Accident Type?

I remember talking to a TapRooT® User about the root causes of bicycle accidents at his refinery. Bicycle accidents were the leading type of accident to cause injuries (imagine that with all the other hazards in a refinery).

Today I ran across this ad:

No chain, enclosed gears, non-slip grips, airless tires (won’t go flat), and less maintenance.

These are all features that would have reduced some of the accidents that had happened at the refinery.

Does it remove all the hazards (energy)? No. But it is a start.
