Interestingly, when you consult several dictionaries for “root cause,” you will get just as many definitions. The result is that people don’t agree on one definition of “root cause.”

When confronted with finding no standard definition for “root cause,” System Improvements Inc. and TapRooT® co-creator Mark Paradies and a colleague, David Busch, developed a first definition as:

The most basic cause (or causes)
that can reasonably be identified
that management has control 
to fix.
