More Bad Root Cause Analysis Advice – This time it’s Pandemic!

OK – I’ll point out another bad piece of advice.

This time a non-profit agency recommended that public health departments use 5-Whys to analyze problems fighting the H1N1 pandemic.

Do they really need to be told to ask why?
