In most old-time movies there was clearly a hero and clearly a villain.

The same is true in most current day major accidents.

The question you have to answer is …

Which one are you going to be?

If you are the villain, you get blamed for the accident. Usually, the blame gets passed down to an operator or a mechanic (someone at the pointy-end of the stick).

Sometimes, if the accident is bigger, the Supervisor or Maintenance Manager gets blamed with the operator or mechanic and they all may face the firing squad.

If the accident is really big and gets the attention of the Board of Directors, even more senior management – perhaps the Plant Manager, may get fired (or transferred to a job with no responsibilities in Timbuktu).

The worst accidents come along with government investigations (the worst – a Presidential Commission). Who knows who will be left after a Presidential Inquisition?

With big accidents, even the people who do the investigation are at risk. If senior management doesn’t like what the investigators have to say, they probably won’t be fired, but their careers could take a big hit.
