Journey Toward Improved Human Performance on A National Patient Safety Goal in a Large Healthcare System

Contributing factors to errors that occur explore the human causes that occurred from which
transformation in health care can occur to sustainable, actionable fixes within the system at a micro or
macro level. Admittedly, it is a complex undertaking in healthcare and more so in corporate systems with numerous unique hospital and clinic setting demands.

However, utilizing aggregated data on a problematic issue from TapRooT® RCAs at a large healthcare
system is a positive movement that occurred. Reactive and proactive human performance improvements were realized on Universal Protocols, which is a Joint Commission best practice to prevent wrong surgeries. Participants will hear applied examples of the journey that continues to occur despite the demands of the complex healthcare environment during the session.
