urgent problem to solve

Are Any of These Problems in Your Top Ten List of Things to Fix?

Here’s the list of potential problems you may be facing:

  1. A string of fatalities (or an increasing trend of serious incidents)
  2. Human error (poor human reliability)
  3. Falling behind other industry leaders in quality
  4. LTI rate higher than the industry average (or trending in the wrong direction)
  5. Poor root cause analysis (management is complaining about the results)
  6. High rate of repeat incidents (ineffective corrective actions)
  7. Blame (people are afraid of telling the truth to investigators)
  8. Too much time spent on investigations/root cause analysis
  9. Management has kneejerk reactions to incidents/incident trends
  10. Repeat equipment failures
  11. No real plan to improve performance

And in the sections below, our ideas to help you fix these types of problems. Just scroll down to your biggest problem and read about ways to solve it.

1. How To Stop Fatalities and Serious Incidents

  1. Implement advanced root cause analysis NOW! (That’s TapRooT® RCA.)
  2. Attack/eliminate your biggest Hazards first. (This assumes you have identified your hazards and ranked them.)
  3. Implement a roadmap to success that continuously improves your performance and your performance improvement efforts.
  4. Move from reactive improvement (reacting to accidents/incidents) to proactive improvement. This starts by identifying precursor incidents and moves on to audits/assessments to manage Hazards and improve performance.

That’s a quick list. Where can you learn more? Here are several articles to read…
